Evolocumab is a recombinant human monoclonal antibody (IgG2 kappa) that specifically binds to and inhibits proprotein convertase subtilisin/kexin type 9 (PCSK9), a protein that regulates LDL receptor degradation in the liver. By blocking PCSK9, Evolocumab increases the number of LDL receptors available on hepatocytes, enhancing the clearance of low-density lipoprotein (LDL) cholesterol from the blood. It is primarily used in the treatment of hyperlipidemia and hypercholesterolemia, particularly in patients who have not achieved desired cholesterol levels through diet and statin therapy alone. This leads to a significant reduction in LDL cholesterol levels, thereby lowering the risk of cardiovascular events such as heart attacks and strokes. It is often prescribed for patients with heterozygous familial hypercholesterolemia (HeFH), homozygous familial hypercholesterolemia (HoFH), or for those with established atherosclerotic cardiovascular disease (ASCVD) who require additional cholesterol management. Administered via subcutaneous injection every two weeks or monthly, it is typically used in combination with other lipid-lowering therapies, such as statins and ezetimibe, for optimal results.
